This Policy Supplement applies only to those Associates working in the state of Arizona, excluding DOT- Drivers . Where state law requires greater or separate requirements than outlined in the Company’s Drug & Alcohol Testing Policy, this Supplement will be followed in accordance with Arizona drug and alcohol testing law and regulations. Drug & Alcohol Testing State Supplement – Arizona

DRUG & ALCOHOL TESTING POLICY – STATE SUPPLEMENT FOR CALIFORNIA

This Policy Supplement applies only to those Associates working in the state of California, excluding DOT- Drivers . Where state law requires greater or separate requirements than outlined in the Company’s Drug & Alcohol Testing Policy, this Supplement will be followed in accordance with California drug and alcohol testing law and regulations. Conditional Offers: Do not allow a Conditional Offer to begin work before the required pre-employment drug test results are received. Advance Warning: All individuals tested should be warned that purchasing medications outside the United States may cause a positive test result. Medical Marijuana

Drug & Alcohol Testing State Supplement – California

DRUG & ALCOHOL TESTING POLICY – STATE SUPPLEMENT FOR CONNECTICUT

This Policy Supplement applies only to those Associates working in the state of Connecticut, excluding DOT- Drivers . Where state law requires greater or separate requirements than outlined in the Company’s Drug & Alcohol Testing Policy, this Supplement will be followed in accordance with Connecticut drug and alcohol testing law and regulations. Advance Written Notice: At the time of application each Conditional Offer will be informed in writing of the Company’s intent to require a drug test. Testing Limitations: Testing may only occur when there are reasonable grounds that the Associate is under the influence of a drug or alcohol which adversely affects or could adversely affect job performance. ( NOTE: an accident by itself is not considered “reasonable suspicion.”) Post-Test Procedures: The Company will provide all current Associates (not Conditional Offers ) with notice of test results and their rights to rebut or explain and their right to independently analyze the sample. The Company will provide the Associate (not Conditional Offers ) with the opportunity, at Associate’s expense , to independently analyze the “split” sample. The Associate must advise the Company of this choice. The Associate will be reimbursed for the cost of the split analysis if it is reported, “not found.” Access to Records: All test related records (including test result) will be made available for copying to Conditional Offer and Associate upon request.

Drug & Alcohol Testing State Supplement – Delaware

DRUG & ALCOHOL TESTING POLICY – STATE SUPPLEMENT FOR HAWAII

This Policy Supplement applies only to those Associates working in the state of Hawaii, excluding DOT- Drivers . Where state law requires greater or separate requirements than outlined in the Company’s Drug & Alcohol Testing Policy, this Supplement will be followed in accordance with the Hawaii drug and alcohol testing law and regulations. Right to Refuse: Each person must be advised at the time of testing that they have a right to refuse. The Company may take employment action for refusing. Advance Written Notice: Each person tested must be given advance written notice specifying the drugs to be tested and that over-the-counter and/or prescribed medicines may cause a positive test result. Collection Sites: The Company will only contract with and allow collection of samples by qualified individuals and in a laboratory qualified/licensed per Hawaii law and regulations. Drug & Alcohol Testing State Supplement – Hawaii

DRUG & ALCOHOL TESTING POLICY – STATE SUPPLEMENT FOR IOWA

This Policy Supplement applies only to those Associates working in the state of Iowa, excluding DOT-Drivers . Where state law requires greater or separate requirements than outlined in the Company’s Drug & Alcohol Testing Policy, this Supplement will be followed in accordance with Iowa drug and alcohol testing law and regulations. Posting Requirement: Confidential access to information for any available Employee Assistance Program services will be posted in a visible location where all other workplace posters are displayed at the worksite location. Advance Written Notice: A copy of the Company policy and this Supplement will be provided to each Associate. The policy will be available for Associates and Conditional Offers to review and copy. Parents of any person under the age of 18 will also be provided a copy. Conditional Offers will also receive written notice of the policy upon hire and before testing. Acknowledgements will be required for each Associate acknowledging that the policy was provided to him/her. Collection Sites: All drug and alcohol test samples will be collected according to state required procedures and analyzed by approved laboratories using authorized approved test methods and technology. Drug test collection will all be made a split sample collection. Post-Incident Testing: Post-accident testing may only occur if it results in a chapter 88 recordable injury or more than $1,000 damage to property. Soft-tissue injuries (e.g. carpal tunnel) are not “accidents” under this policy. Post-Test Procedures: For any positive test result the Company will send by certified mail, return receipt requested , written notice of the result to the Associate and information regarding the Associate’s rights to have the “split” portion of the sample tested at a lab of the Associate’s choosing. Any Associate who is under the age of 18 and the Associate’s parent/guardian will be notified by certified mail, return receipt requested of any positive test result and the Associate’s rights. Consequences: The disciplinary provisions of the Company policy will be followed. However, anyone with an alcohol result of 0.04 or above who has worked for the Company 12 of the past 18 months will be offered an opportunity at counseling. If no other benefit program is available, the Company will assist with the cost of treatment. Access to Records: All test related records (including test result) will be made available for copying to Conditional Offer and Associate upon request. Required Training: Same as Company policy but supervisors will at a minimum receive an initial 2 hour and annual 1 hour training concerning the recognition of employee alcohol abuse and drug use, how to document it, how to corroborate it and how to appropriately make referrals for assistance.

All Iowa Associates must be given a copy of the Company’s Drug & Alcohol Testing policy along with this State Supplement and sign the Drug & Alcohol Testing Consent Form.

Drug & Alcohol Testing State Supplement – Iowa

DRUG & ALCOHOL TESTING POLICY – STATE SUPPLEMENT FOR LOUISIANA

This Policy Supplement applies only to those Associates working in the state of Louisiana, excluding DOT- Drivers . Where state law requires greater or separate requirements than outlined in the Company’s Drug & Alcohol Testing Policy, this Supplement will be followed in accordance with the Louisiana drug and alcohol testing law and regulations. Collection Sites: All drug and alcohol test samples will be collected according to state required procedures and analyzed by approved laboratories using authorized approved test methods and technology. Drug test collection will all be made a split sample collection. Post-Incident Testing: The laboratory will preserve a sufficient portion of the sample for retesting if the test is positive. Access to Records: Any Associate testing positive will, within 7 working days , have access to any record related to the test, including records related to certification, review, or suspension/revocation-of-certification proceedings.

Drug & Alcohol Testing State Supplement – Louisiana

DRUG & ALCOHOL TESTING POLICY – STATE SUPPLEMENT FOR MARYLAND

This Policy Supplement applies only to those Associates working in the state of Maryland, excluding DOT- Drivers . Where state law requires greater or separate requirements than outlined in the Company’s Drug & Alcohol Testing Policy, this Supplement will be followed in accordance with the Maryland Health-General Code Ann. §17-214 et seq . and Admin. Code §10.10.10.01 et seq . Testing Limitations: NO BREATH-ALCOHOL TESTING ALLOWED. Otherwise all collection and laboratory analysis will be in accordance with Maryland state regulations. The permitted type of specimen obtained for testing varies between Conditional Offer and current Associate. See table below. Type of Specimen Allowed Conditional Offer Current Associate

Collection Sites: At the time of testing the person being tested will be informed of the name and address of the laboratory that will test the specimen. Post-Test Procedure: Within 7 days of receiving any positive test result the Company will by hand or certified mail deliver to either a Conditional Offer or Associate the following: 1. A copy of the laboratory test indicating the test results; 2. A copy or written summary of the employer's policy covering an Associate or Conditional Offer with a confirmed positive test result; 3. If applicable, written notice of the employer's intent to take disciplinary action, terminate employment, or change the conditions of continued employment; and 4. A statement or copy of the provisions set forth in §B of this regulation permitting a Conditional Offer , Associate, or contractor to request independent testing of the same sample for verification of the test result. Retest Procedure: A Conditional Offer or Associate who is required to submit to an alcohol or drug test may request that an independent test be performed on the same specimen in a confirmation test by: 1. Notifying the Company and the laboratory that performed the test of the challenge to that test result; and 2. Requesting that laboratory to submit a sufficient portion of the original specimen to a different laboratory chosen by the Conditional Offer or Associate and properly licensed to perform alcohol or controlled dangerous substances testing. 3. The Conditional Offer or Associate must select an independent testing laboratory that will perform the independent test using detection levels for alcohol or controlled dangerous substances equal to or lower than those provided by the laboratory that performed the initial confirmation test. 4. The Conditional Offer or Associate will pay all costs of independent testing including any costs associated with specimen handling and transport. Medical Marijuana

This Policy Supplement applies only to those Associates working in the state of Minnesota, excluding DOT- Drivers . Where state law requires greater or separate requirements than outlined in the Company’s Drug & Alcohol Testing Policy, this Supplement will be followed in accordance with the Minnesota Drug and Alcohol Testing in the Workplace Act . Posting Requirement: Notice of Company policy and this Supplement will be posted in a visible location where all other workplace posters are displayed at the worksite location. Advance Written Notice: Written notice of this policy will be provided to each Associate. Conditional Offers will also receive written notice of the policy upon hire and before testing. Copies of Policy Available: Copies of the policy will be made available by management during regular business hours. Conditional Offers and Associates may receive a copy the policy by requesting from management during normal business hours. Right to Refuse: Each Conditional Offer and Associate has the right to refuse any requested test. But, be aware that disciplinary action may occur if one refuses to test. Each Conditional Offer and Associate must acknowledge being provided the policy or given access to the policy. Collection Sites: All drug and alcohol test samples will be collected according to state required procedures and analyzed by approved laboratories using authorized approved test methods and technology. Post-Test Procedures: Within 3 working days any Conditional Offer or Associate will be notified in writing of their test result, their right to request a retest and the timing of that right, the right to explain the result and the limitation on “first-time-positive” discipline and what their options are. Consequences: No Associate testing positive the first time may be discharged unless that Associate refuses or fails to complete counseling. Access to Records: All test related records (including test result) will be made available for copying to Conditional Offer and Associate upon request.

Drug & Alcohol Testing State Supplement – Minnesota

DRUG & ALCOHOL TESTING POLICY – STATE SUPPLEMENT FOR MONTANA

This Policy Supplement applies only to those Associates working in the state of Montana, excluding DOT- Drivers . Where state law requires greater or separate requirements than outlined in the Company’s Drug & Alcohol Testing Policy, this Supplement will be followed in accordance with the Montana Workforce Drug and Alcohol Testing Act . Testing Limitations: All testing conducted will be in compliance with the federal Department of Transportation “DOT” rules and is limited only to those Associates engaged in the performance, supervision, or management of work in a: 1. hazardous work environment (as defined below); 2. security position; or 3. position: a. affecting public safety or public health; b. in which driving a motor vehicle is necessary for any part of the individual's work duties; or c. involving a fiduciary responsibility for an employer. "Hazardous work environment" includes but is not limited to positions: (1) for which controlled substance and alcohol testing is mandated by federal law, such as aviation, commercial motor carrier, railroad, pipeline, and commercial marine Associates; (2) that involve the operation of or work in proximity to construction equipment, industrial machinery, or mining activities; or (3) that involve handling or proximity to flammable materials, explosives, toxic chemicals, or similar substances. Sample for testing includes urine, breath or oral fluid. Reasonable Suspicion Testing: If there is reason to suspect that an Associate’s faculties are impaired on the job as a result of controlled substance or alcohol use, the Associate may be tested. Post-Incident Testing: Any Associate involved in a workplace accident resulting in a death, injury or property damage in excess of $1,500, and there is reason to believe the Associate may have caused or contributed to the accident will be subject to drug and/or alcohol testing. Post-Test Procedures: The Company will provide an Associate tested with a copy of the test report. Retest: The Company will also provide, at the Associate’s request a test of the “split sample” by an independent laboratory selected by the Associate. The Company will pay for the test if the result is negative (or not found present) and the Associate will pay if the split sample result is positive (or found present). Rebuttal: The Associate will be provided an opportunity to rebut any test result and advised of the process for initiating that rebuttal opportunity. (e.g. who to call/notify-phone number, in writing, etc.) Confidentiality: All information related to a drug or alcohol test, including interviews, reports, statements, memoranda and test results are considered confidential and will be treated as such. Only authorized individuals will have access to such materials. Required Training: Supervisors will receive appropriate training in accordance with federal DOT requirements.

DRUG & ALCOHOL TESTING POLICY – STATE SUPPLEMENT FOR NORTH CAROLINA

This Policy Supplement applies only to those Associates working in the state of North Carolina, excluding DOT-Drivers . Where state law requires greater or separate requirements than outlined in the Company’s Drug & Alcohol Testing Policy, this Supplement will be followed in accordance with North Carolina drug and alcohol testing law and regulations. Under North Carolina law, the person requiring a drug or alcohol test is referred to as an “examiner” and the person being tested is referred to as the “examinee.” For clarification within the policy, an examiner is identified as the Company and an examinee is identified collectively as the Conditional Offer /Associate. Advance Written Notice: At the time of submitting to testing, the Company will provide the Conditional Offer /Associate with written notice of their rights and responsibilities. Collection Sites: The Company may choose to conduct on-site testing of Conditional Offers using FDA- approved, single-use devices; existing Associates will only be tested in an approved Collection Site . All testing will be conducted in accordance with North Carolina law. Drug Screening Cut-Off Levels that differ from the Company Drug Screening Standard : Type of Drug Initial Test Cut-Off Level Confirmatory Test Cut-Off Level Amphetamines (meth) 1,000 ng/mL 500 ng/mL Cocaine 300 ng/mL 150 ng/mL The Company may add substances permitted by law. Post-Test Procedures: Within 30 days after a positive test result is received by the Company, the Company will give written notice to the Conditional Offer /Associate of the positive result and their rights and responsibilities regarding retesting. Retest Procedures: A Conditional Offer /Associate with a reported positive test result has the right to a retest. A Conditional Offer /Associate can waive this right or choose to have the result retested. If retesting is chosen, the following steps will be followed: 1. The Company/original lab will follow the instructions of the retesting lab. 2. The Conditional Offer /Associate must request a retest in writing specifying an approved lab to conduct the retest. 3. The Conditional Offer /Associate must pay all “reasonable costs” of a retest which include: a. The actual costs of the retest; b. Fees associated with the retest including chain-of-custody and shipping; c. No more than $15 for the Company’s costs unless the Company can prove the costs are greater than $15; and d. The actual cost of shipping the sample to retest. Access to Records: All test related records (including test result) will be made available for copying to Conditional Offer /Associate upon request. All North Carolina Associates must be given a copy of the Company’s Drug & Alcohol Testing policy along with this State Supplement and sign the Drug & Alcohol Testing Consent Form.

Drug & Alcohol Testing State Supplement – North Carolina

DRUG & ALCOHOL TESTING POLICY – STATE SUPPLEMENT FOR NEBRASKA

This Policy Supplement applies only to those Associates working in the state of Nebraska, excluding DOT- Drivers . Where state law requires greater or separate requirements than outlined in the Company’s Drug & Alcohol Testing Policy, this Supplement will be followed in accordance with the Nebraska Workplace Drug and Alcohol Testing Law . Collection Sites: The Company will only contract with and allow testing for alcohol using devices and personnel appropriate under state law and drug testing will be conducted in a laboratory qualified/licensed per Nebraska law. Access to Records: All test related records (including test result) will be made available for copying to Conditional Offer and Associate upon request. All Nebraska Associates must be given a copy of the Company’s Drug & Alcohol Testing policy along with this State Supplement and sign the Drug & Alcohol Testing Consent Form.

DRUG & ALCOHOL TESTING POLICY – STATE SUPPLEMENT FOR NEVADA

This Policy Supplement applies only to those Associates working in the state of Nevada, excluding DOT-Drivers . Where state law requires greater or separate requirements than outlined in the Company’s Drug & Alcohol Testing Policy, this Supplement will be followed in accordance with Nevada drug and alcohol testing law and regulations. Collection Sites: All testing will be conducted in a state-approved laboratory. Medical Marijuana: Nevada state law allows the use of marijuana for specific medical purposes. The law does not: 1. Require the Company to pay for or reimburse a person for costs associated with the medical use of marijuana. 2. Require the Company to accommodate or allow the medical use of marijuana in the workplace. 3. Require the Company to modify the job or working conditions of a person who engages in the medical use of marijuana but the Company will attempt to make reasonable accommodations for the medical needs of an Associate who engages in the medical use of marijuana if the Associate holds a valid registry identification card, provided that such reasonable accommodation would not: a. Pose a threat of harm or danger to persons or property or impose an undue hardship on the Company; or b. Prohibit the Associate from fulfilling any and all of his or her job responsibilities. All Nevada Associates must be given a copy of the Company’s Drug & Alcohol Testing policy along with this State Supplement and sign the Drug & Alcohol Testing Consent Form.

Drug & Alcohol Testing State Supplement – Nevada

DRUG & ALCOHOL TESTING POLICY – STATE SUPPLEMENT FOR NEW YORK

This Policy Supplement applies only to those Associates working in the state of New York, excluding DOT- Drivers . Where state law requires greater or separate requirements than outlined in the Company’s Drug & Alcohol Testing Policy, this Supplement will be followed in accordance with New York drug and alcohol testing law and regulations. This Policy Supplement applies only to those Associates working in the state of Rhode Island, excluding DOT- Drivers . Where state law requires greater or separate requirements than outlined in the Company’s Drug & Alcohol Testing Policy, this Supplement will be followed in accordance with Rhode Island drug and alcohol testing law and regulations. Advance Written Notice: At the time of application each Conditional Offer will be informed in writing of the Company’s intent to require a drug test. Testing Limitations: Testing may only occur when there are reasonable grounds that the Associate is under the influence of a drug or alcohol which adversely affects job performance. ( NOTE: an accident by itself is not considered “reasonable suspicion.”) Post-Test Procedures: The Company will provide all current Associates (not Conditional Offers ) with notice of test results and their rights to rebut or explain and independently analyze the sample. The Company will provide the Associate (not Conditional Offers ) with the opportunity, at Company expense , to independently analyze the sample. The Associate must advise the Company of this choice. Consequences: An Associate testing positive the first time will not be terminated and will be referred for assistance to a substance abuse professional licensed in Rhode Island (licensed physician, certified psychologist, social worker, Employee Assistance Program professional or NAADAC-certified substance abuse counselor). Anyone with a second positive test result will be terminated. Access to Records: All test related records (including test result) will be made available for copying to Conditional Offer and Associate upon request.

DRUG & ALCOHOL TESTING POLICY – STATE SUPPLEMENT FOR VERMONT

This Policy Supplement applies only to those Associates working in the state of Vermont, excluding DOT- Drivers . Where state law requires greater or separate requirements than outlined in the Company’s Drug & Alcohol Testing Policy, this Supplement will be followed in accordance with Vermont drug and alcohol testing law and regulations. Advance Written Notice: At the time of application each Conditional Offer will be informed in writing of the Company’s intent to require a drug test. The Conditional Offer will also be told that the list of drugs to be tested can be found in the Company policy and advised that no therapeutic levels of prescribed medicines will be reported to the Company. Testing Limitation: Testing may only occur when there is probable cause to believe that the Associate is using or under the influence of a prohibited substance while on the job. Post-Test Procedures: All Conditional Offers and Associates will be provided with notice of test results and their rights to rebut or explain and their right to independently analyze the sample. The Company will provide the Associate (not the Conditional Offer ) with the opportunity, at Company expense , to independently analyze the sample. The Associate must advise the Company of this choice. Consequences: Any Associate testing positive the first time will not be terminated and will be referred for assistance to the Company’s Employee Assistance Program. Anyone refusing to participate in counseling, failing to successfully complete counseling or with a second positive test result will be terminated. Suspension: Anyone agreeing to counseling may be suspended for the period of counseling, but in no event longer than 3 months Employee Assistance : Any Associate who needs assistance with a drug or alcohol issue should contact the Human Services office.

POLÍTICA DE EXÁMENES DE DETECCIÓN DE DROGAS Y ALCOHOL: SUPLEMENTO ESTATAL PARA IOWA Este Suplemento estatal se aplica únicamente a los asociados que trabajan en el estado de Iowa, excluidos los conductores del Departamento de Transporte . Si la ley estatal estipula requisitos mayores o distintos de los estipulados en la Política de Exámenes de Detección de Drogas y Alcohol de la empresa, se cumplirá este Suplemento de conformidad con las leyes y normas de exámenes de detección de drogas y alcohol de Iowa. Requisito de publicación: El acceso confidencial a información sobre los servicios disponibles del Programa de asistencia para empleados se publicará en un lugar visible donde se publican todos los demás afiches laborales en el lugar de trabajo. Notificación previa por escrito: Se proporcionará una copia de la política empresarial y de este Suplemento a todos los asociados. Sin embargo, las personas con un resultado de examen de alcohol de 0,04 o superior que hayan trabajado en la empresa 12 de los últimos 18 meses tendrán la oportunidad de recibir ayuda profesional. Si no se ofrece otro beneficio del programa, la empresa ayudará con el costo del tratamiento. Lugares para la toma de muestras: Todas las muestras para los exámenes de drogas y alcohol se tomarán de conformidad con los procedimientos estatales obligatorios y se analizarán en laboratorios aprobados con el uso de tecnología y métodos de examen aprobados y autorizados. POLÍTICA DE EXÁMENES DE DETECCIÓN DE DROGAS Y ALCOHOL: SUPLEMENTO ESTATAL PARA MARYLAND Este Suplemento estatal se aplica únicamente a los asociados que trabajan en el estado de Maryland, excluidos los conductores del Departamento de Transporte . Si la ley estatal estipula requisitos mayores o distintos de los estipulados en la Política de Exámenes de Detección de Drogas y Alcohol de la empresa, se cumplirá este Suplemento de conformidad con el Código de Salud General §17-214 y siguientes y sus modificaciones y el Código Administrativo §10.10.10.01 y siguientes . Limitaciones a los exámenes: NO SE PERMITEN LOS EXÁMENES DE ALCOHOL EN EL ALIENTO. Procedimiento de repetición de exámenes: Un empleado con oferta condicional o asociado que deba someterse a un examen de detección de drogas o alcohol puede solicitar que se realice un análisis independiente de la misma muestra en un examen de confirmación mediante: 1. la notificación a la empresa y al laboratorio que realizó el examen sobre el desafío al resultado de dicho examen; y 2. la solicitud al laboratorio de que envíe una parte suficiente de la muestra original a un laboratorio diferente, escogido por el empleado con oferta condicional o el asociado y con la licencia correspondiente para realizar exámenes de detección de sustancias peligrosas controladas. 2. 3.
